Upon scheduling an appointment with our office you will be sent a secure link via text or email containing online forms. All online forms are secure and HIPAA compliant.

Prior to your visit, please complete the online forms from your mobile device or computer. You will be able to fill out a health history form, sign policy/consent forms, upload your insurance card, authorize another person (other than a parent/legal guardian) to attend an appointment with your child and much more!

If you choose to use a paper form, simply print and complete the form below.

Your Safety is Our Concern

Infection control has always been a top priority for our practice and you may have seen this during your visits to our office. Our infection control processes are made so that when you receive care, it’s both safe and comfortable. Our office follows infection control recommendations made by the American Dental Association (ADA), the U.S. Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration (OSHA).

For the well-being and safety of our patients and staff, if your child is not feeling well, please call to change your appointment. Additionally, if you or anyone in your household is sick or has had Covid in the last 7 days, please call our office to reschedule.

Two-way text messaging with our staff for all non-medical related matters (our text line: 855-324-5982).
Mobile app to complete online paperwork, confirm & check in for visits, complete secure online payments, update your insurance/contact info, customize your reminders, and more. Download the app here.
Payment transactions over the phone (and online)
Family-based care: Families of 2 or more may request that their children be seen one after another or all children to be seen at the same time with a different provider. We encourage parents/guardians to come into the dental room during treatment, however, you may opt to wait in the waiting room or in your car. If a child will be coming alone/without a legal guardian, please complete the Guardian Authorization Form or text us the word CONSENT and we will text you the form to complete securely on your mobile device.

Please inform us of an updated address, phone number and/or insurance information during your check-in at the front desk.
